Please provide a written request including the following information:

• Your name (as it was when you attended CHS)
• Your birth date
• Year of graduation
• Address for where to send the transcript or if you will be picking it up.
• YOUR SIGNATURE
• A pone number to reach you if there are questions.

Please indicate whether the transcript needs to be official or unofficial copies.

Hand deliver the above written request to the Counselor's Office or mail to Centennial High School, Attn: Transcript Requests, 12400 W. McMillan Road, Boise, ID 83713

There is a $2.00 fee for each copy that is mailed or picked up.
There is a $5.00 fee for each copy that is faxed.

Transcript requests are processed once a week, normally on Thursday each week.
